Caravan Park Defib Subsidy Application Form 

The following form is used to apply for the subsidisation of a defibrillator and should be read in conjunction with the introduction page which specifies eligibility and funding requirements. All fields are required and please note that any missing information may lead to delays in the payment of the subsidy.  If you have any questions regarding the below form or the application process, please contact defibs@caravanindustry.com.au

  • I agree that myself and/or responsible staff will have been or will be appropriately trained in the use of the defibrillators by a competent authority, have accepted full responsibility for and all liabilities relating to the defibrillators’ including and not limited to their placement within the caravan park that I am claiming the subsidy for including their security, upkeep and maintenance, and any ongoing training. I also agree that the defibrillator being subsidised is located on the Australian Register of Therapeutic Goods.
  • On submission of the Caravan Park Defibrillator Subsidy Program, I agree to fully indemnify the Caravan Industry Association of Australia Limited and the Commonwealth of Australia, in relation to the subsidised defibrillators.
  • All expired defibrillators must be disposed of in accordance with Section 108A of the Product Stewardship Act 2011. Incorrectly disposing defibrillators, specifically their batteries, can lead to a significant fire risk.
  • Submission and Next Steps

    On submission of this application form, you agree that you have the relevant authority on behalf of the business/organisation submitting this application and that you have the relevant authority to consent to the terms and conditions of the subsidy program. Once submitted, Caravan Industry Association of Australia will follow up with you to inform you of the outcome of the application, collect relevant bank details and to arrange payment of the subsidy.
